History of Present Illness
The patient is an 82-year-old female who presented with progressive difficulty in breathing, bilateral lower limb swelling, exertional dyspnoea, paroxysmal nocturnal dyspnoea, and intermittent chest pain. The symptoms had gradually worsened over the past few weeks, limiting her daily activities and exercise tolerance.

Known history of hypertension, on antihypertensive medication although adherence and specific medications are not immediately available
